Southcentral Foundation’s Document Library


Over the decades Southcentral Foundation has been providing health care, SCF has regularly documented procedures and best practices. SCF uses these documents internally to support the provision of care, but SCF’s Learning Institute has also made many of these documents available for other organizations to download through our Document Library.

SCF started the Document Library during the first year of the COVID pandemic in order to share documents related to COVID safety protocols, vaccination, and more with other health care organizations. Since then, SCF has expanded the Document Library to include documents on a wide variety of other topics. The library is divided into four major categories:

  • Clinical Operations. These documents provide information on how SCF runs its primary care clinics, including information on appointments, case management, customer-owner check-in, customer-owner satisfaction surveys, Integrated Care Team roles and responsibilities, and more.
  • Workforce Support and Human Resources. These documents provide information about SCF’s workforce practices, including guidelines for remote work, academic leave, self-care leave, video conferencing, and more.
  • Integrated Care Team Job Descriptions. These example job descriptions provide information about responsibilities, necessary qualifications, the working environment, and physical demands for the following integrated care team positions at Southcentral Foundation: physician, RN case manager, certified medical assistant, case management support, and behavioral health consultant.
  • SCF’s Aging Well Initiative. These documents provide information on Aging Well, which is a system-wide initiative that builds off the foundation of robust services and resources SCF already has in place to optimize care for Elder customer-owners. 

Also, SCF has published an Empanelment Supplement to its Integrated Care Teams playbook, and both are now available as a single document through the Learning Institute’s website.
